Patients are referred to our hospital from either within or outside institutions or relevant organizations. Upon arrival, our center initiates case management and arranges interdisciplinary integrated outpatient services. The outpatient team consists of members such as cardiologists, pediatric cardiologists, electrophysiology specialists, cardiac geneticists, genetic counselors, and pediatric medical counselors, among others, depending on the patient's condition. Genetic testing for patients is arranged during outpatient assessment, followed by further testing or family genetic testing based on the results. Our center plans to develop educational materials and health passports for various genetic heart diseases. This not only enables patients and their families to understand their own and their relatives' illnesses but also provides other healthcare professionals caring for the patients with accurate and effective focal points for care. Case managers, genetic counselors, and pediatric medical counselors assist patients and their families throughout the various processes of treatment, ensuring that the formulation and implementation of integrated treatment plans are conducted under conditions that meet the physical and mental health needs of the individual and their family.
