Application Form for the National Taiwan University Medical Library (NTUML) Library Card for Alien Medical Staff in Training at National Taiwan University Hospital (NTUH)

In case of any violations, I will accept any penalties according to the National Taiwan University Library Patron Penalty Rules. Before leaving the NTU Hospital I will complete the departing procedures by returning the library card and clearing any outstanding fines and replacement charges.
