“Patient-centered Seamless Transition and Referral System (P-STARs)”

The first stage of the NTUH P-STARs involves contracting with cooperated healthcare organizations, establishing a web platform to share information and promoting the visions and goals of P-STARs among stakeholders. Currently, the P-STARs web platform can send instant SMS notification of referral and the sharing of relevant information from the hospital doctor to the community doctor, referral appointment making, communication note taking and messaging among physicians, which builds the bridge to link with different healthcare facilities and our medical center.

 

NTUH launched the first contrast ceremony with 419 healthcare organizations on 5th October, 2018. To propagate the idea of P-STARS, thirty lectures were given to NTUH departments, and four consensus conferences were held for participants outside NTUH, including P-STARs Annual Convention and Contract Conference.

 

Just after the launch of NTUH P-STARs in early of 2019, there were 2000 referrals monthly to our hospital, contrast to less than 100 outpatients and less than 20 inpatients referrals a month to other healthcare facilities. Not until the end of 2019, NTUH has received more than 3,000 outpatient referrals per month from our healthcare alliance organizations. Most notably, the NTUH had referred over 1,400 patients a month to our community healthcare alliances organizations, including more than 200 inpatients after discharge. Recently, we have launched the second year of NTUH P-STARs contract ceremony. There are a total of 461 healthcare organizations participate P-STARs healthcare networks in 2020-21. Four P-STARs consensus meeting and education programs will be held annually.

 

The second stage of the NTUH P-STARs focuses not only on integrating outpatients and referral case management but also on promoting the publicity of our alliance healthcare organizations and educating people the knowledge and consensus of “Choosing Wisely”. In 2020, NTUH plans to promote integrated care clinics and case management system to meet the patients’ need by reducing polypharmacy, and doctor shopping. We aim to provide quality care for high medical utilizer and palliative patients. In the meantime, “Choosing Wisely” educational seminars and consensus building conference will be held in NTUH healthcare alliance networks.
