X-Ray Image Data Application

Office hours (collection of data on the same day):
Monday ~ Friday: 8:00~16:00.

Office hours (collection of data on the following day):
Monday ~ Friday: 16:00~18:00.

Application venue: Registration Office, Department of Medical Imaging, NTUH West Site (No. 1 Changde Street, Examination Building)

  • Note 1: Those intending to apply for hard copies of bone mass measurement data from the Department of Nuclear Medicine shall first fill in the “NTUH Medical Records Summary and Data Copy Application Form” at the Medical Records Office, NTU West Site. Subsequently, they can present the application report at the Department of Nuclear Medicine registration desk to receive the copies.
  • Note 2: Those applying for dental X-ray images shall first confirm with the Department of Dentistry that the images have been uploaded, after which they can receive a CD-copy of the X-ray images from the Department of Dentistry.
  • Note 3: For dental CT images and images from the Cardiovascular Center, please apply at the institution of where the examination was originally performed.
  • Note 4: If part of a set of ultrasound (e.g. heart ultrasound), endoscopy, or eye examination data cannot be received, please apply for a written report at the Medical Records Office.

If allowed by CD capacity, all images can be copied to one CD. However, the fees will differ depending on the CD type.

Fee regulations:

  • General (fee: NT$200): No restriction on the number of copies; if allowed by CD capacity, all images can be copied to one CD.
  • Special:
    Single copy: NT$200
    Two copies: NT$400
    Three copies (incl.) or more: NT$500

If, more than one CD is required, NT$100 is charged for each additional CD.

Application procedure: 

  1. Fill in the application form.
  2. Have the documents verified by the person in charge.
  3. Make the payment.
  4. Receive the copies.

Medical records are private. Please understand that, in order to protect the patient's interests, the applicant will not be allowed to receive the copies if his or her application documents are incomplete.

Data and Applicant Required Documents

Nationals of other countries must provide original copies of the residence card and passport.

Application via an agent
  1. Original copy of the patient's identification document.
  2. Original copy of the agent's identification document.
  3. Authorization Consent Form signed by the patient.

An authorization consent form from the Department of Psychiatry must be signed and sealed by the patient. Foreign nationals must provide original copies of the residence card and passport.

Application via a legal representative (applicable to minors; as stipulated by Article 12 of Civil Code, citizens are considered adults upon reaching 20 years of age) (guardian)
  • Original copy of the patient's identification document (the original copy of a household certificate must be provided for minors)
  • Original copy of the legal representative's identification document
  • Documented proof of relation to the patient (original copies of household certificate, court decision, etc.)
If application is made via an agent, the aforementioned data, Authorization Consent Form, and an original copy of the agent's identification document must be prepared.

Foreign nationals must provide original copies of their residence card and passport.
Application via the holder with the right of inheritance
(application for data related to the deceased)
  • Original copy of the inheritance right holder's identification document.
  • Documented proof of relation to the patient (original copies of identification document, household certificate, etc.)
  • Original copy of the certificate confirming the removal of the patient's name from the registered residence (household cancellation certificate or death certificate).
If the application is done via an agent, the aforementioned data, Authorization Consent Form, and original copy of the agent's identification document must be prepared.

Foreign nationals must provide original copies of the residence card and passport.

 

Copy collection time:

  1. In principle, X-ray and imaging data can be collected after 60 minutes. However, if the imaging data are excessively large, more time may be needed to prepare a CD. The collection time will be rescheduled with the applicant.
  2. Data that cannot be provided on the same day can be sent to an address within Taiwan by mail (for a NT$50 fee paid by the applicant).
  3. The processing of applications for data from the Department of Nuclear Medicine may start three days after the completion of medical examinations and report delivery by the physician to ensure data integrity.

Other matters:

  1. Inpatients should submit applications to the ward secretary.
  2. Some special images (e.g. CT and NMRI conducted on the same day) require post-processing. Please start the application seven days after completion of the examination and report delivery by the physician in order to ensure data integrity.
  3. The processing of applications for data from the Department of Nuclear Medicine may start three days after completion of medical examinations and report delivery by the physician to ensure data integrity.
  4. For dental CT images and images from the Cardiovascular Center, please apply at the institution where the examination was originally performed.
  5. If parts of a set of ultrasound (e.g. heart ultrasound), endoscopy, and eye examination data cannot be received, please apply for a written report at the Medical Records Office.
  6. Medical records are private. Please understand that, in order to protect the patient's interests, the applicant will not be allowed to receive the copies if his or her application documents are incomplete.
