Hepatitis B and C Case Management Team
Our hospital is dedicated to providing comprehensive care for hepatitis patients by establishing a specialized Hepatitis Case Management Team. This team assists individuals with hepatitis B, hepatitis C, and other chronic liver diseases by managing their condition. Our goal is to ensure that each patient receives the highest standard of medical care.
Service Scope:
1. Establish standardized case management procedures to remind patients of scheduled follow-up appointments and monitor liver ultrasound and blood test results, improving patient follow-up rates.
2. Provide expert guidance on hepatitis medications to enhance treatment adherence, prevent improper medication use, and ensure optimal therapeutic outcomes.
3. Facilitate seamless continuity of care by coordinating the transfer of hepatitis medications between healthcare facilities.
4. Contact individuals who test positive during hepatitis screenings, provide medical consultations, and facilitate treatment referrals to improve early diagnosis and intervention.
5. Provide a dedicated telephone service line to address general inquiries regarding liver diseases.
Furthermore, our team collaborates with the National Health Agency and the National Health Insurance Administration (NHIA) to implement hepatitis prevention and management programs aimed at increasing screening accessibility and evaluating the effectiveness of hepatitis care.
I. Expanded Adult Preventive Health Screening for Hepatitis B and C
In alignment with the national policy to eradicate hepatitis C, our hospital’s Family Medicine and Internal Medicine departments offer expanded adult preventive health screenings for hepatitis B and C. Individuals who meet the eligibility criteria, as determined by their National Health Insurance (NHI) card, are entitled to a one-time free hepatitis B and C screening.
Eligibility Criteria:
- General population aged 45 to 79 years
- Indigenous population aged 40 to 79 years
Screening Services Include:
- Hepatitis B surface antigen (HBsAg) blood test
- Hepatitis C antibody (Anti-HCV) blood test
II. National Health Insurance Program for Hepatitis B and C Management and Treatment Support
To enhance follow-up care and treatment adherence among hepatitis B and C patients, the NHIA has been implementing this initiative since 2010. The objective is to mitigate the risk of liver cancer by promoting regular monitoring and early diagnosis, thereby ensuring that patients do not miss critical treatment opportunities.
Eligibility Criteria:
- Individuals diagnosed with chronic hepatitis B or C, regardless of whether they are currently undergoing treatment.
Services Provided:
- Regular monitoring of liver function through blood tests and abdominal ultrasound examinations.
- Liver disease education and individual case management.
- Telephone follow-ups for patients who have discontinued treatment to assess potential barriers to care and provide appropriate referral support.
Enrolment in the Case Management and Follow-Up Program
Patients may make an appointment at the Gastroenterology and Hepatology Clinic within the Internal Medicine Department. If the physician determines that the patient meets the eligibility criteria and agrees to regular follow-ups, they may enroll in the program. A dedicated case manager will then provide individualized care and treatment recommendations.
