Blood and Marrow Transplant Case Management Team
Before undergoing hematopoietic stem cell transplantation (HSCT), patients must undergo a comprehensive physiological and psychological assessment by various medical specialists. During the treatment process, patients receive high-dose chemotherapy and, in some cases, radiation therapy. Allogeneic HSCT recipients, in particular, require immunosuppressive therapy, which increases the risk of complications such as acute or chronic graft-versus-host disease (GVHD) and infections. These complications can range from mildly affecting the patient's quality of life to being life-threatening.
To provide more comprehensive and specialized care for HSCT patients, National Taiwan University Hospital established the “Blood and Marrow Transplant Case Management Team” in March 2020. The team comprises two dedicated Blood and Marrow Transplant (BMT) coordinators who support both HSCT recipients and their related donors.Our services include full-spectrum medical management, personalized consultation, and long-term post-transplant follow-up to ensure a smooth transplantation process and enhance patients' quality of life and survival rates.
Job Responsibilities:
- Provide Pre-, Peri-, and Post-Transplant Care: Offer nursing education to recipients throughout the transplant process, conduct regular follow-ups and documentation, and deliver holistic care to both patients and their families.
- Integrate Multidisciplinary Resources: Coordinate pre- and post-transplant evaluations, integrate interdisciplinary medical resources, assess urgent medical needs, and facilitate appropriate treatment processes and referrals.
- Support Related Donors: Perform assessments and provide nursing education for related donors, coordinate donation preparations and schedules, and manage post-donation follow-up care.
- Engage in Multidisciplinary Meetings: Participate in team meetings to stay informed about care plans, assist physicians in explaining treatment plans to recipients, identify high-risk transplant cases, and organize family meetings and pre-transplant briefings.
- Monitor Compliance: Follow up with recipients who miss scheduled visits, offer support, analyze reasons for non-compliance, and report findings to the medical team.
- Educational Initiatives: Organize transplant education activities and participate in teaching sessions, including clinical teaching, seminars, and patient support group meetings.
