Telehealth Care Manager

The NTUH Telehealth Center provides professional instruments for remote physiological measurements, including blood pressure, blood glucose, EKGs, and blood oxygen. These instruments allow patients to take measurements in their own homes. The remote monitoring system transmits recorded measurements directly to the hospital. Professional case managers are responsible for case follow-up, providing around-the-clock, personalized health service and management to assist in managing the health care team is continuously aware of patients’ health conditions. The team provides personalized health services and management according to patients’ different needs. By continually tracking patients’ physical conditions, hospital travel times and emotional costs can be reduced, thereby improving quality of life.
