Service Target

The majority of cases are from asthma patients, and others include patients or medical staff who prescribe inhaled medications.

Service Content

Case admission and follow-up

1. Decide whether to accept the patient as an asthma disease management case based on the provisions of the National Health Insurance Administration's "Asthma Medical Benefit Improvement Program" and the patient's wishes. After receiving the case, the patient's basic information will be filled in first and the health education test paper will be taken.

2. Newly admitted patients will receive eight comprehensive health education courses, including [Pathological mechanisms of asthma], [How to diagnose asthma], [Understanding the severity of asthma], [Allergens and environmental control], [Handling of acute attacks], [How to control asthma in the long term], [Asthma drug education], and [The importance of monitoring PEF].

3. Teach the correct way to inhale the drug and when to use it, and confirm the correct method on the spot.

4. Measure the patient’s peak expiratory flow rate (PEF) and calculate the green, yellow and red light zones based on the patient’s estimated value. In addition, fill out the Asthma Control Test (ACT), and use the PEF and ACT values as the basis for evaluating the condition and degree of improvement.

5. New patients who return for the first visit will fill out the post-education test paper and the medication response form. Each patient who returns for a consultation will have their peak expiratory flow (PEF) measured according to the conditions, and will complete an asthma control test (ACT). The patient will be

asked about their symptoms from the last visit to the current visit, health education related issues for the patient, and environmental control recommendations based on the season. Depending on the situation, the patient's medication inhalation method will be repeatedly confirmed to be correct. Finally, the data will be entered into the case management system to provide doctors with information about the patient's symptoms, PEF and ACT

6. Develop and maintain an asthma database and case management system

 

Phone Tracking

For patients who do not return for regular checkups, the disease control specialist will proactively call to inquire about the patient's current situation and ask why they did not return for checkups. Depending on the situation, they will provide telephone health education and assist in registering for return checks. When we receive calls from registered patients for consultation, we will provide them with health education guidance over the phone and assist them in making follow-up appointments if necessary.

 

 

Other Services

Services are also provided to patients who are not on the physician's caseload or are not on record. The content includes: teaching how to inhale medicine, teaching how to blow PEF and record, filling out ACT (Asthma Control Test), and other asthma-related knowledge.
