Oncology Case Management

Service Recipients:

  • Newly diagnosed cancer patients. 

  • Cancer patients undergoing active treatment at our hospital after initial local recurrence or single organ metastasis. 

Service Location

  • Oncology-related outpatient clinics and various wards, with telephone consultation available. 

Service Hours

  • Monday to Friday, 8:00 AM to 5:00 PM. 

Consultation Method

  • Directly contact the responsible case manager. 

Service Content

  • Provide continuous care, including nursing guidance and regular follow-up, based on the individual needs of oncology patients, and offer a single point of contact for consultation via official telephone. 

  • Assess the healthcare needs of cancer patients and provide patient-centered, holistic, family-oriented, continuous, and comprehensive care. 

  • Participate in multidisciplinary team meetings to understand patients' care plans, assist physicians in explaining the treatment and care plans at each stage to patients, and help resolve issues. 

  • Serve as a communication and coordination channel between other medical team members, assisting in resolving the health needs of cancer patients and their families. 

  • Provide emotional support and care to patients and their families when facing diagnosis and treatment, and provide timely referrals as needed. 

  • Responsible for telephone follow-up and consultation for cancer patients who have not received treatment or have interrupted treatment after diagnosis.
